Program Overview

Family Centers Healthcare seeks a full-time experienced, energetic, and compassionate Advanced Practice Nurse Practitioner (FNP or PNP) or Physician Assistant (Primary Care) to work as part of a dynamic, multi-disciplinary team to deliver comprehensive primary medical care to patients. This opportunity would combined primary care skills in both an outpatient setting and within a school setting in a School Based Health Center.

Family Centers Healthcare is a Federally Qualified Health Clinic (FQHC) located in Greenwich and Stamford. The clinic provides primary healthcare, dental and mental health services to those living in Greenwich and Stamford communities and the surrounding neighborhoods.

The Role

Incumbent will be responsible for the diagnosis and treatment of common and chronic illnesses, planning of periodic health assessments, vaccines, screenings, promoting optimal health and preventative care. Additional responsibilities include community referrals, accurate and comprehensive documentation, providing patient education, and other regulatory compliance and training requirements.

Qualifications And Educational Requirements

• Master’s or post-Master’s degree from an accredited program and at least 2 years of relevant experience
• Demonstrated aptitude in providing medical services largely geared to under-served populations.
• Current active license to practice in the State of Connecticut
• Medical malpractice insurance (in addition to that offered by the agency)
• Current active DEA license
• Ability to work flexible schedule including day and early evening hours
• Bilingual - Spanish preferred

Family Centers is a United Way, Community Fund of Darien and New Canaan Community Foundation partner agency, and is a member of the Connecticut Council of Family Service Agencies and the Alliance for Children and Families. The agency is also accredited by the National Association for the Education of Young Children (NAEYC), The Joint Commission, licensed by the State of Connecticut Department of Public Health, and is a Department of Children and Families-licensed child psychology center.
