San Luis Valley Health is seeking a full-time Discharge Planner in our Labor & Delivery department. Reporting to the Care Coordination Manager, the Discharge Planner will also have computer knowledge in Excel and Microsoft applications, apply critical thinking, and be able to effectively communicate with hospital staff, patients and families, and community members. This position prioritizes support for high-risk patients, particularly those at-risk for substance use. Spanish speaking is a plus.

What You'll Need:

  • Education/degree in Social Work field or Nursing, preferred.
  • Minimum of two years of care coordination related experience, preferred.
  • Minimum of two years of direct healthcare or social services experience, required, or education equivalent.
  • Computer knowledge in Excel, Microsoft Word, Microsoft Access and Internet, preferred.
  • Effective critical thinking and verbal skills to communicate with hospital staff, external agencies, and patients and families, required.
  • Bilingual preferred.

What You'll Do:

  • Work closely with Women's Health Care Coordinator and other care coordinators, community partners, and discharge planning team at SLVH.
  • Responsible for maintaining effective communication with all team members, family, patient, and other medical staff throughout our organization.
  • Attends regular meetings with the team and departments to discuss the patients’ condition, progression, plans, and goals.
  • Work hand in hand with physician(s) and other team members regarding specific request and/or suggestions.
  • Communicate with Medical Staff, primary care nurse and other hospital staff regarding discharge planning process prior to discharge for needs of the patient and family members.
  • Demonstrates the ability to respond to the social needs of various cultural, ethnic, and religious groups.
  • Responsible for hospital discharge screening and assessments, planning, and developing Plans of Safe Care when appropriate.
  • Ensures the Social Services function complies with all regulatory requirements, provide support/counseling to patients and family members, and develop and/or present educational programs to hospital staff to assist them in identifying the psychosocial needs of the patients.
  • Participates in community committees or functions that are relevant in the networking, outreach programs as needed.
